Gym Waiver and Release of Liability

ALPHA PRO CENTER BOXING GYM WAIVER AND RELEASE OF LIABILITY

In consideration of being allowed to participate in boxing training, sparring, and related activities at Alpha Pro Center Boxing Gym, I, Gym Member * agree to the following waiver and release of liability.

I acknowledge that boxing is a contact sport and that there is a risk of injury, including serious injury or death, associated with participation in boxing-related activities.

I understand and acknowledge that the Alpha Pro Center Boxing Gym cannot guarantee my safety and that I participate in boxing-related activities at my own risk.

I agree to assume all risks associated with participation in boxing-related activities at the Alpha Pro Center Boxing Gym, including but not limited to the risk of injury, death, or property damage.

I agree to follow all rules and regulations of the Alpha Pro Center Boxing Gym and to obey all instructions given by its trainers and staff.

I agree to release, hold harmless, and indemnify the Alpha Pro Center Boxing Gym, its owners, employees, and agents from any and all claims, damages, and expenses, including legal fees, arising from my participation in boxing-related activities at the Alpha Pro Center Boxing Gym, whether caused by the negligence of the Alpha Pro Center Boxing Gym or otherwise.

I certify that I am physically fit to participate in boxing-related activities and that I have no medical conditions that would prevent me from doing so.

I acknowledge that this waiver and release of liability is binding on my heirs, assigns, and legal representatives.

I HAVE READ AND UNDERSTAND THIS WAIVER AND RELEASE OF LIABILITY AND AGREE TO BE BOUND BY ITS TERMS.
